Tennessee Department of Correction Commissioner Frank Strada has officially resigned following a colossal bureaucratic failure that left convicted killer Christa Pike alive after two separate failed lethal injection attempts.

Tennessee Governor Bill Lee announced Saturday that Strada will leave his post this month as the state launches an independent investigation into what went wrong inside Riverbend Maximum Security Institution on Wednesday night.

“Commissioner Strada has served with integrity, and I appreciate his willingness to put the interests of Tennesseans first during this difficult moment,” Lee said in a statement reported by USA Today.

“I have accepted his decision to step down and am grateful for his commitment to ensuring a smooth leadership transition. I thank Commissioner Strada for his dedicated service to Tennessee and pray God’s blessings over him in the days ahead.”

Strada, in a statement released through the governor’s office, said stepping aside “is in the best interest of Tennesseans as the independent review moves forward.”

He called the review “entirely appropriate and necessary” and said he would “do everything I can to help determine exactly what occurred,” according to the New York Post. No replacement has been named yet.

Pike, 50, was supposed to be the first woman executed in Tennessee in more than 200 years. Instead, executioners administered two full doses of pentobarbital on Wednesday night and she survived.

As The Gateway Pundit reported, her attorneys told a federal court she had not lost consciousness, still had a heartbeat, and was audibly snoring inside the death chamber.

Witnesses described her gasping and asking whether the burning in her arm was “normal” before officials pulled the blinds. An ambulance rolled up while media witnesses were already at the podium.

By Friday she was unconscious, intubated, and on a ventilator at a Nashville-area hospital, her lawyers said in a court filing cited by CBS News.

Hospital staff were trying to clear two doses of the execution drug from her body. She has significant injuries to both arms. Her attorneys wrote that the IV team apparently never realized the lines were not correctly placed or that her veins had blown, so the pentobarbital was entering her tissue instead of her bloodstream, according to the Los Angeles Times.

The Tennessee Department of Correction insisted it “followed every step” of the protocol approved by the attorney general’s office and that the protocol does not allow further procedures beyond what was carried out. Pike was then transported off-site.

Lee had already called the failure “deeply disturbing,” ordered a comprehensive third-party review, and canceled the state’s remaining 2026 execution, as The Gateway Pundit reported Thursday.

Strada was hired in 2023 to restart Tennessee executions after a 2022 independent review found the state had for years failed to follow its own lethal-injection rules. He came from Arizona, where he was a top corrections official during three lethal injections later criticized for serious problems, including a case in which the team resorted to a femoral line, The New York Times reported.

Since he arrived in Tennessee, the department has now mishandled two executions in 2026. In May it failed to execute Tony Carruthers, convicted of killing three people.

Republican State Sen. Tom Hatcher, a former chairman of the corrections oversight committee, told the Times that if he rated commissioners from one to 10, Strada “would be a one.”
